    Client Service Agreement & Terms and Conditions



    1. Purpose

    This Service Agreement sets out the terms under which Liight Me Up Again Services (the Provider) delivers services to the Client/Participant. It outlines expectations, responsibilities, and mutual commitments to ensure ethical, professional, and culturally safe support.

    2. Scope of Services

    Liight Me Up Again Services provides non-clinical support, including:

    • Consultation
    • Social Work (non-clinical)
    • Career Development and Leadership Coaching
    • Relationship Coaching
    • Personal Development & Mindset Coaching
    • NDIS Support Coordination
    • Psychosocial Recovery Coaching
    • Self-Guided Programs
    • Public Speaking

    Note: These services are for personal growth, empowerment, and education. They are not a substitute for therapy, clinical diagnosis, or medical treatment.

    3. Eligibility

    • Clients must be 18 years or older, unless written parental/guardian consent is provided.
    • NDIS participants must have self-managed, plan-managed, or NDIA-managed funding.
    • International clients acknowledge services are governed by the laws of South Australia, Australia and are non-clinical.
    • In such cases, Liight Me Up Again Services may notify appropriate services (e.g., emergency services, crisis teams, or statutory bodies) to ensure safety.
    • Notes and records are securely stored, and confidentiality continues after services end.

    4. Confidentiality & Privacy

    • All sessions are confidential in line with the Australian Privacy Principles (APPs).
    • Information will only be shared with consent, except when disclosure is required to protect safety or by law, including:
      • Risk of suicide, self-harm, or harm to others
      • Disclosure required by law, regulation, or court order
      • There are child protection or vulnerable adult concerns.
    • Notes and records are securely stored, and confidentiality continues after services end.

    5. Responsibilities

    Provider Responsibilities

    • Deliver services professionally, ethically, and within agreed scope.
    • Support the Client/Participant to explore goals and strategies.
    • Clarify that services are non-clinical, not therapy, and not medical advice.

    Client Responsibilities

    • Be honest and engaged in the process.
    • Take responsibility for their own choices, actions, wellbeing, and outcomes.
    • Inform their mental health provider if under care.
    • Understand that Liight Me Up Again Services provides guidance, not guarantees.

    Disclaimer: The Client remains fully responsible for their own life and decisions. Coaching and social work support are designed to empower and guide, but do not replace therapy, medical treatment, or crisis services.

    6. Responsibilities

    Provider Responsibilities

    • Deliver services professionally, ethically, and within scope.
    • Communicate openly and respectfully.
    • Clarify that services are not therapy, diagnosis, or crisis care.
    • Provide invoices, reports, and updates (including NDIS requirements).

    Client Responsibilities

    • Provide accurate and up-to-date information.
    • Participate actively in sessions and agreed actions.
    • Treat staff respectfully and maintain boundaries.
    • Accept responsibility for their own wellbeing, decisions, and outcomes.

    7. Booking & Payment

    • Sessions are pre-booked and prepaid unless otherwise agreed in writing.
    • Accepted payments: Stripe, bank transfer, invoicing (NDIS).
    • International clients are charged in AUD unless agreed otherwise.
    • NDIS-funded supports are billed in line with the NDIS Pricing Arrangements and Price Limits.

    8. Cancellations & Refunds

    • Non-NDIS services: Minimum 24 hours’ notice is required.
    • NDIS-funded services: Minimum 7 days’ notice is required.
    • Late cancellations may be charged the full fee.
    • Refunds are discretionary and reviewed case by case.

    9. Code of Conduct

    • All parties agree to respectful, ethical, and safe behaviour.
    • Services may be suspended or ended if safety, respect, or ethical boundaries are violated.

    10. Disclaimers

    • No outcomes are guaranteed.
    • Results vary depending on client engagement.
    • Services are for empowerment, education, and growth — not therapy or medical treatment.

    11. Intellectual Property

    • All materials, resources, and program content belong to Liight Me Up Again Services.
    • Reproduction or distribution requires written consent.

    12. Jurisdiction

    • This Agreement is governed by the laws of South Australia, Australia.
    • International clients accept South Australian jurisdiction.

    13. NDIS-Specific Terms (if applicable)

    • Services will be delivered in line with the Participant’s NDIS Plan goals.
    • Clients consent to the Provider liaising with NDIA, Plan Managers, and other providers as needed.
    • Reports, invoices, and communications will be provided as required by the NDIS Quality and Safeguards Commission.

    14. Conflict of Interest Disclosure

    • A potential conflict of interest may arise if a participant chooses both NDIS-funded services and private coaching/consultation.
    • To manage this:
      • NDIS services will be billed only according to the NDIS Price Guide.
      • Private services will be offered under a separate agreement.
      • Clients have full choice and control to use another provider if they wish.

    15. Cultural Safety & Inclusion

    • Liight Me Up Again Services acknowledges and respects the cultural identity, traditions, and connections to Country of Aboriginal and Torres Strait Islander peoples.
    • We value the diversity of CALD (Culturally and Linguistically Diverse) communities and provide culturally responsive support.
    • Interpreters or culturally appropriate support can be arranged if required (charges may apply).
    • We are committed to delivering services that are culturally safe, inclusive, and respectful.

    16. Termination, Fees & Disputes

    • Either party may end this Agreement with 14 days’ written notice.
    • Immediate termination may apply for safety, ethics, or compliance issues.
    • Prepaid sessions may be refunded pro-rata (minus delivered services/admin fees).
    • Disputes should be raised in writing and will be addressed within 14 business days.

    17. Complaints & Feedback

    • Clients are encouraged to raise concerns directly with Liight Me Up Again Services.
    • If unresolved, NDIS participants may contact:

    18. Client Acknowledgement & Consent

    • I have read, understood, and agree to the Terms and Conditions.
    • I understand these services are non-clinical and not a substitute for therapy or medical care.
    • I understand my responsibilities, including attendance, participation, and communication.
    • I consent to services being delivered under this Agreement.
